Public or private institutions/organizations may request for the verification of the scholastic records of current and former students of the UP Open University. Verification covers all the students’ records in the university – graduates, continuing, honorably dismissed, transferred, cross-enrollee, non-degree and certificate students. The requesting party should provide the OUR an authorization letter/consent from the student to release his/her record, otherwise, no action will be made by the OUR. Generally, 3 to 5 working days from receipt of the request is allotted in processing the verification request.
- Verification form of the verifying institution.
- Duly signed authorization of the student for the release of his/her university records.
- Verification fee payment receipt amounting to Php 50.00. View the payment procedures here.
- Courier fee payment receipt amounting to PhP 190.00 if the verifying institution wishes to secure the original copy of the verified form. View the payment procedures here. If the scanned copy of the verified form is sufficient, courier fee is not needed.

Step 2: Wait for an email from the Office of the University Registrar for the result of the request.
